Please Read & Understand Then Complete Waiver

Lazer Kraze provides great, fun activities including world class multi-level laser tag, Cyber Sport and an Indoor Trampoline park. These are physical activities and as such have inherent risks of injury. All participants play at their own risk.

In order to jump on the indoor trampoline park all participants must have a signed waiver. For jumpers under 18, it must be signed by the PARENT or COURT APPOINTED LEGAL GUARDIAN. It can NOT be signed by aunts, grandparents, scout leaders, etc. It must be the PARENT or COURT APPOINTED LEGAL GUARDIAN that signs the waiver for the minor.
